I agree that if I misrepresent or leave out important facts on this form, then Bowers + Kubota can refuse to employ me, or if I am employed, would be grounds for immediate dismissal. I also certify that I filled this application out myself, and I understand everything on this application.

I consent to and authorize Bowers + Kubota to make a full and complete investigation of my personal or employment history and authorize any former employer, person, firm, corporations, school, credit agency, government agency or other entity to provide the Company with any information of any sort (including fact or opinion) they may have regarding me. In consideration of the Company’s review of this application, I release the Company and all providers of any information from any liability as a result of furnishing and receiving this information.

I understand and agree that an investigative consumer report may be made concerning my character, reputation, personal characteristics and mode of living. I hereby consent to and authorize that such a report be made which may include information regarding my credit. Information as to the nature and scope of this report may be obtained upon request.

I understand and agree that this application is not a contract and that acceptance of employment is not a contract of employment for a specified term. I understand and agree that I may resign my employment with Bowers + Kubota at any time for any reason, and that my employment may be terminated at the will of Bowers + Kubota at any time for any reason without liability of any kind. I also understand that any handbooks, manuals, policies, and procedures maintained by Bowers + Kubota are not contractual in nature and may be amended or abolished at the sole discretion of Bowers + Kubota at any time.

I understand, pursuant to the Immigration Reform and Control Act, Bowers + Kubota will employ only those individuals who are eligible to work in the United States. Accordingly, upon hiring, I understand that I will be required to demonstrate my eligibility to work in the United States. Failure to do so will result in termination or revocation of the offer of employment.

If employed by Bowers + Kubota, I agree to conform to the guidelines and policies of the Company including but not limited to Bowers + Kubota’s Code of Ethics and Standards of Conduct. I will report all suspected violations of law related thereto, and will conduct the company’s business in a strictly ethical and legal manner. Furthermore, I acknowledge that Bowers + Kubota has established a Drug-Free Awareness Program. Should I seek employment with Bowers + Kubota in which pre- employment drug testing has been implemented, I acknowledge that I will be required to pass a drug screening test as a condition of employment. Should I become an employee of Bowers + Kubota, I will abide by the terms of Bowers + Kubota’s Drug Abuse Policy and related management instructions. I will, in addition, obey all of the laws of the United States and of all localities, states, and nations where Bowers + Kubota does business.

I agree to all of the consents, authorizations and releases which I have made on this application shall be irrevocable during the period of my employment should I be hired by Bowers + Kubota. I also understand and agree that all of the foregoing terms and conditions will become part of my employment agreement with Bowers + Kubota if I am employed by the Company.
